Frequently asked questions
What families ask us most.
Select any question to reveal its answer. If your situation is different, our care team can help you find the right starting point.
What services does Community Outreach Advocates provide?
We support individuals and families through home-based services, community transition support, caregiver and family training, family and life coaching, stress and time-management coaching, and home medical equipment assistance. Our team can help you understand which option may fit your situation.
Can a family member be paid to care for a loved one?
Structured Family Care may provide eligible family caregivers with compensation, training, and ongoing resources for the daily support they already provide at home. Eligibility depends on the program requirements and your individual circumstances, so our team will review the next steps with you.
How can I find out whether I am eligible?
Start by sharing your basic contact information and preferred way to be reached. A team member can then explain the applicable program requirements and guide you toward the right intake path. An initial inquiry does not guarantee enrollment or benefits.
Can you help someone transition home from a facility?
Our Community Transition Services are designed to support a safer return home from a hospital, rehabilitation center, or nursing facility. Support may include transition planning, home-safety preparation, equipment assistance, daily-living support, and coordination with appropriate resources.
Do you help with home medical equipment?
Yes. We can assist families with appropriate mobility and home-care equipment, setup and usage guidance, coverage navigation, maintenance, and replacement support. Equipment and coverage availability depend on each person's needs and program eligibility.
Do family caregivers receive training and support?
Our caregiver and family training is designed to build confidence with daily routines, care plans, communication, stress management, and sustainable caregiving habits. Structured Family Care participants may also receive ongoing coaching and program resources.
Do you work with families and healthcare providers?
Yes. We partner with families and appropriate healthcare providers to coordinate person-centered support while respecting consent, privacy, and each client's preferences.
What should I include in an online inquiry?
Only provide the requested contact details and your preferred contact method. Please do not submit diagnoses, treatment details, insurance information, or other private health information through the public inquiry form. Our team will follow up through the contact channel you select.
